Debating between angular-schema-form and angular-formly... Any preference, I am leaning towards angular-formly.

This involved the UI filtering capabilities on the 3 fields mentioned (gender, severity, and product_type)

@patricknowlan... any status on this one. I'm trying to close out issues for v0.4 as today is the firm cutoff for any code commits
