Need HTML Form for Recording EMS Events #5

You should, for now, hard code all of the CORONAVIRUS EVENTS.

The form should contain, in order, the following:

hidden input type with the name/id "ems_id".
Date. Of type "date" in HTML5.
Time, regular text box just big enough to fit "00:00", using type "time".
EVENT. A dropbox containing:
1. Cardiac Arrest
2. Heart Problem / Chest Pain
3. Unconscious Person
4. Syncope
5. Choking
6. Breathing Problem
7. Intense Headache
8. Sick Person
9. Stroke
Street 1
Street 2: Same line as Street 1
Notes: small <textarea>
