"TAP version" missing #64

This should output "TAP version 12" at the beginning, as some TAP parsers won't parse without that

@palfrey, TAP parsers should not accept TAP version 12. From the specification:

In the absence of any version line version 12 is assumed. It is an error to explicitly specify any version lower than 13.

You should take the issue up with the TAP parsers that require version 12 to be specified.
