Allow both a static value & percentage for payment fee

Currently it's only possible to either set a static value, or a percentage for the payment fee. It would be great if these could be individually set (ie: AND instead of OR).
Reason being that buckaroo applies both a static fee and a percentage of the order amount for many payment methods; the plugin should reflect this.
