Enhancement: Integration with InterSystems Server Manager

It would be great if this could tap into the same set of server definitions managed by the InterSystems Server Manager. That may be less trivial than it sounds, as it's the main SQLTools extension that seems to manage these and not this driver, but I thought logging it here wouldn't hurt.

I think this is doable but would require a small enhancement in the main SQLTools extension which the IRIS driver would then leverage. I'm in a good position to do both ends of this as I'm now a maintainer of the main extension.
