PhenomeCentral playground should be skinned to always be a playground #22

The playground looks just like the regular page, but should not.

  • It should always show 'playground' prominently on every page.
  • The registration page should probably NOT have the following fields:
    • "HOW DID YOU HEAR ABOUT / WHO REFERRED YOU TO PHENOMECENTRAL?"
    • "WHY ARE YOU REQUESTING ACCESS TO PHENOMECENTRAL?"
    • "I CONFIRM THAT I AM A DOCTOR, CLINICAL GENETICIST OR SCIENTIST WORKING WITH PATIENTS AFFECTED BY RARE DISORDERS."
