…tory Six coded columns listed values that exist in no value set, four of them on required bindings: provider_role, accepting_new_patients, affiliation_type, plan_type, location_type and sex. Every documented code is now verified present in the code system Plan-Net binds. facility_type_nucc and specialty_nucc kept their NUCC codes but linked OrgTypeVS and a raw taxonomy expansion; both now link the value set the profile actually binds. Restores the Data conventions table dropped in 65ce92a, which is what explains why there is no column for active or status.
