Notifying plugins of volume changes in a player was added with version 3 of the plugin API. The actual call was never implemented though.
The new plugin API version lets plugins select playlist entry by index. It's a bit odd to use `mstarTrackVolumeChanged` for the volume of the CD Player when there is a dedicated function for the player volume itself. Fix that odd behavior in the new API version.
