This PR adds new plugin structure originally used only for Copilot CLI: https://docs.github.com/en/copilot/how-tos/copilot-cli/customize-copilot/plugins-creating now being used also by VS Code (with Github extension) https://code.visualstudio.com/docs/copilot/customization/agent-plugins
This approach drops nesting plugin content under .github folder. This also solved some of the issues with loading MCP configuration in VS Code.
