Fix Starship and Herdr dotfile defaults

Summary

Fixes Starship and Herdr dotfile defaults so the Cortex terminal setup comes back with the expected palette, Herdr defaults, and remote-session helpers.

Changes

• Restores the Cortex Starship palette and prompt symbol color.
• Tunes Herdr terminal defaults: terminal theme plus Cortex custom colors, managed SSH config, hidden single-tab tab bar, and disabled sound.
• Adds hremote-stop and hremote-restart helpers for stale remote named sessions.
• Removes duplicate Herdr install entries.
• Notes that the review gate verdict was persisted for diff sha daebae1beac4d1c828da728ed327663f7bf6cb0013e4c3105cb090b940938231.

Testing / validation

• Formal reviewer and security-reviewer returned no findings for the final diff.
• review-gate evaluate returned allow.
