Add GitHub adoption kit

• add a copy-paste GitHub adoption kit with a BugBundle policy and Issue Form
• add a feature-request Issue Form and optional reviewed-bundle field to this repository

• link the public five-project design-partner pilot from the README

Why

BugBundle's next validation gate is real adoption in external Node.js repositories. Maintainers need a low-friction, safety-conscious integration they can review and copy without designing their own policy or issue workflow.

Impact

The CLI and bundle format are unchanged. Maintainers now have a concrete adoption path, and BugBundle's own issue workflow supports feature proposals and dogfoods reviewed bundle attachments.
